Job Description-Paraprofessional (Community Support Worker)


Essential Functions of the Position:

Under the direct supervision of a Qualified Mental Health Professional.

v Provide psycho-educational activities for high risk intervention consumers. Activities include social skill training, management and monitoring of behavioral point systems, behavioral contracting, contract resolution and skill training, anticipatory guidance, life space interviewing, and personal care instruction. Activities are implemented as outlined in consumer service plan prepared by treatment team or consumer’s case manager.

v Provide close supervision of consumer throughout assigned area.

v Maintain accurate consumer information regarding goals, interventions, service plans and significant incidents and submit all documentation/progress notes.

v Participate in all mandatory clinical and administrative meetings regarding service implementation and consumer progress and maintain communication with supervisory staff regarding consumer issues and service delivery.

v Review and function in accordance with the policies and procedures of Fonseca’s Family Services LLC. And complete all agency training.

v May perform the following activities:

a) Skill building including daily and community living skill; socialization skills, adaptation skills, development of leisure time interests/activities, symptom management skills; wellness education and work readiness; education substance and behavior and anger management techniques. Perform other duties as assigned.





Minimum Qualifications Required:
Graduate from high school or GED, at least 18 years of age, able to read, write, understand and follow directions; Or no GED or high school diploma, employed prior to November 1, 2001 to provide a mh/dd/sa service. Supervision shall be provided by a qualified professional or associate professional with the population served. The supervisor and the employee shall develop an individualized supervision plan upon hiring. The parties shall review the plan annually.

Other Requirements:

Must a have valid North Carolina Driver’s License.
