Job Description-Associate Professional
Essential functions of position:
· Provide an array of services; including treatment plans and case management to specialized caseload of corrections clients, adult and juvenile, including dual-diagnoses clients, occurring in custody and/or in the community.
· Coordinate and oversee the initial and ongoing assessment activities, initial development and ongoing revision of person centered plan; monitoring of implementation of plan.
· Coordinate with community agencies to carry out the treatment plan.
· Provide direct services as indicated in the treatment plan.
· Provide support, counseling and education to consumers to assist them in achieving their recovery goals.
· Assist consumers in accessing available benefits and services.
· Implement approaches that utilize and enhance consumers’ natural supports.
· Coordinate service plans with involved family, caregivers, medical, psychiatric and other provider.
· Utilize motivational interviewing and provides stage appropriate interventions.
· Maintain all applicable professional, legal and ethical standards, including confidentiality, dual relations, and informed consent.
· Provide documentation, reports, and records to meet State, Federal, Regional and departmental standards and requirements: Clear, organized, timely progress notes.
Other Functions:
1. Work effectively within a multidisciplinary team.
2. Attend and participate in staff meeting, community agency meetings and skill trainings/updates.
3. Complete other assignments and tasks as directed by supervisor and/ or Director.
Minimum Education and Training:
(a)graduate of a college or university with a Masters degree in human service field with less than one year of full-time, post-graduate degree accumulated mh/dd/sa experience with the population served, or a substance abuse professional with less than one year of full-time, post-graduate degree accumulated supervised experience in alcoholism and drug abuse counseling. Supervision shall be provided by a qualified professional with the population individualized supervision plan upon hiring. The Parties shall review the plan annually; or
(b) graduate of a college or university with a bachelor’s degree in a human service field with less than two years of full-time, post-bachelor’s degree accumulated mh/dd/sa experience with the population served, or a substance abuse professional with less than two years of full-time, post-bachelor’s degree accumulated supervised experience in alcoholism and drug abuse counseling. Supervision shall be provided by qualified professional with the population served until the individual meets two years of experience. The supervisor and the employee shall develop an individualized supervision plan upon hiring. The parties shall review the plan annually: or
(c) graduate of a college or university with a bachelor’s degree in a field other than human services with less than four years of full-time, post-bachelor’s degree accumulated mh/dd/sa experience with the population served, or a experience in alcoholism and drug abuse counseling. Supervision shall be provided by a qualified professional with the population served until the individual meets four years of experience. The supervisor and the employee shall develop an individualized supervision plan upon hiring. The parties shall review the plan annually; or
(d) Registered nurse who is licensed to practice in the State of North Carolina by the North Carolina Board of Supervision shall be provided by a qualified professional with the population served until the individual meets four years of experience. The supervisor and the employee shall develop an individualized supervision plan upon hiring. The parties shall review the plan annually.
Other requirements:
Must have a valid driver’s license; will be required to be supervised by a QMHP until such time that the above staffing requirements have been met.
